Collaborating and Communicating to Build an Integrated Health Care Model: Join together with key stakeholders (physicians, psychiatrists, families, consumers, insurers) to learn about and discuss the integration of medical and behavioral health care. The ever-evolving nature of health care knowledge and technology demands an equally ever-changing response to how it should be organized, delivered and paid. The environment dictates renewed focus on building a continuum of care that supports American health care consumers throughout their lifespan. The evolving needs of American society and health care consumers, impacted by demographic trends, work-life trends, advances in health care technology and treatment, access to information and other factors is impacting the ability of health care providers to effectively meet needs in an independent health care delivery model. Rapidly increasing costs are forcing policymakers, providers, businesses, consumers and their families to find new ways to deliver more cost-effective healthcare. Driven by these factors and the growing complexity of today's environment, cooperation, coordination and collaboration are a prerequisite for effective health care delivery.
