Upcoming changes to Medicaid may impact access to care for Granite Staters in the coming months and years. The recently passed federal budget reconciliation bill includes drastic cuts to Medicaid, with nearly $1 trillion in spending reductions over the next ten years. Additionally, this legislation includes several new Medicaid policies to be implemented in the coming years, including establishing work requirements for Medicaid enrollees, requiring cost-sharing for enrollees of expansion programs, mandating twice-yearly eligibility review, and shortening the retroactive coverage window for new enrollees to 30 days. Further, the 2026-2027 NH State Budget includes similar changes to Medicaid, with new premiums, increased cost-sharing, and work requirements.

Though some provisions of the federal budget reconciliation bill take effect upon passage, the implementation of other policies may not occur for 2-4 years. New Hampshire’s Medicaid premiums are set to begin in July 2026, and though the state budget requires NH DHHS to submit a waiver to implement work requirements by January 2026, it is unclear when those requirements would come into effect. These changes, combined with the lack of clarity surrounding implementation timelines, have the potential to make it more difficult for Medicaid enrollees in New Hampshire to maintain coverage and access care – which is why it’s crucial to stay up to date and reach out with any questions you may have.
